Please bring your photo ID, current insurance card(s), and copay or amounts due. Payments must be made on the day you’re seen, or we will have to reschedule your appointment.

We accept credit cards and cash.

We accept all major insurance plans including, but not limited to: Aetna, Blue Cross Blue Shield, Cigna, GEHA, Healthsmart, Humana, Medicare, Medicare Advantage/Replacement plans, Oscar, Scott and White, Tricare, TriWest, and United Healthcare. If you insurance isn’t on this list, please call us at 817-348-8600, we likely take your insurance too!

If you are receiving anesthesia or having a procedure done, please have a responsible driver (18+ years old) take you home.
